What IS 1st FIVE?
1st Five is a public-private partnership bridging family practice providers and pediatricians with public health services.
The 1st Five Program supports family practice providers and pediatricians in the earlier detection of social-emotional and
developmental delays and family risk-related factors in children birth to 5 and coordinates referrals, interventions and follow-up.
HOW can 1st FIVE help you?
1st Five can connect a family to a resource in their community. 1st Five helps families by following up with a phone call or a home
visit. 1st Five will keep your family practice provider or pediatrician informed on what resources your family has utilized.
We can follow up with a phone call, home visit and assistance with referrals to services in the community and surrounding communities.
We will keep provider informed of progress being made with the child and family.

Provider recognizes a developmental delay in child age 0-5 or notes family issues that may affect mental well-being of child.
Referral is sent to Warren County Health Services to follow up with family.
1st Five coordinator contacts family to discuss issues they are facing and services that would be beneficial.
Referrals are made to community services
1st Five coordinator keeps provider updated on progress with services and referrals made
